Please use the Forms Tab above to complete/submit your online consent form prior to arriving for your appointment. If it is your first visit to our Spa, all new clients must have an electronic version of the “New” Massage or Skin Care form completed, based on the service you are receiving. On all returning visits, you will simply complete the “Returning” Massage or Skin Care form. A consent form must be completed for each visit.

Pre-Care Waxing Recommendations
• Let hair grow out at least 2-3 weeks, or about 1/4 inch before waxing.
• Exfoliate the areas that will be waxed 1 day before your appointment.
• Do not apply any body lotion on the day of your appointment. The skin needs to be dry and free of any moisturizing products.

Post-Care Waxing Recommendations
• Begin exfoliating the area waxed 48 hrs after to prevent ingrown hairs.
• A warm to cool temperature shower/bath is recommended for the next 24 hours.
• You are more sensitive before your period, schedule waxing a few days after.
• Schedule your next appointment in 4-6 weeks for easy maintenance.

Please arrive freshly showered for all Bikini/Brazilian services • Stay out of the sun/tanning beds 48 hours before & after your waxing appointment

	Waxing – Underarms $25 or 250 Points Earned Points: 25 Waxing of both underarms
	Waxing – Bikini Line $45 or 450 Points Earned Points:45 Bikini Line Wax
	Waxing – Full Bikini $60 or 600 Points Earned Points: 60 Full Bikini Wax
	CurlPerfect™ Eyelash Lift  – Enhance Your Natural Lashes $95 or 950 Points Earned Points: 50 CurlPerfect™ Eyelash Lift is a low-maintenance lash perm that lifts/curls and enhances your natural lashes for up to 6 weeks! The Lash Lift curls, sets, and conditions natural lashes to make it look as if you used a really good eyelash curler. We also tint your lashes black to enhance the results. Please arrive with all eye makeup and contact lenses removed *Clients with Lash Extensions cannot have the Lash Lift procedure, as to not interfere/breakdown the extensions*
	Waxing – Brow Shaping and cleanup $20 or 200 Points Earned Points: 20 Keep your brows looking their best with our brow shaping and cleanup service.
	Eyelash Tint $25 or 250 Points Earned Points: 25 Enhance & darken your own lashes to create a fuller lash line and accentuate your eyes. Add-on a Brow Tint for only $15.00 more. Treatment tint usually lasts 3-4 weeks, but results will vary.
	Waxing – Lips $10 or 100 Points Earned Points: 10 Upper lip wax
	Waxing – Lip and Chin $15 or 150 Points Earned Points: 15 Lip and Chin wax
	Waxing – Full legs $90 or 900 Points Earned Points: 90 Complete waxing of both the upper and lower legs.
	Waxing – Half legs $45 or 450 Points Earned Points: 45 Half Leg Wax
	Waxing – Brazilian $75 or 750 Points Earned Points: 75 Brazilian Wax. Please arrive freshly showered.
