Please use the Forms Tab above to complete/submit your online consent form prior to arriving for your appointment. If it is your first visit to our Spa, all new clients must have an electronic version of the “New” Massage or Skin Care form completed, based on the service you are receiving. On all returning visits, you will simply complete the “Returning” Massage or Skin Care form. A consent form must be completed for each visit.

	Revive Signature Massage – 60 Minutes $135 or 1200 Points Earned Points: 60 Whether you like a lot of pressure (Deep Tissue) and want those problem knots worked out, or just want a relaxing massage (Swedish) to de-stress, our experienced staff will design a session to meet your needs. 	Hot Stone Massage – 90 Min $185 or 1800 Points Earned Points: 90 A delightful warm treatment using Basalt Stones to rub away tension. The stones are perfectly heated and used throughout the massage to penetrate deep into the muscle. You’ll leave feeling Revived! *At least 2 Hour advance scheduling notice is required to allow the stones to be heated to the correct temperature
	Pregnancy – 60 Minutes $135 or 1200 Points Earned Points: 60 Becoming a mother is very exciting, but it can also be very stressful on the body. Prenatal massage is a great complement to reduce the normal discomforts that are associated with pregnancy such as back pain, headaches, joint pain and disturbed sleep patterns. Recipients must be past their 1st trimester.
	Thai Massage – 90 Minutes $185 or 1800 Points Earned Points: 145 Thai Massage helps restore balance and the natural energy flow of the body. The therapist uses their hands, elbows, knees, feet and entire legs to move the client into a series of yoga stretches. The client is dressed and should wear loose comfortable clothing (long athletic pants and t-shirt).
	Tune Up – 30 Minutes $70 or 600 Points Earned Points: 30 Had a crazy week at work or just feeling stressed? Try a 30 minute tune up which is performed on a massage table and focuses on the upper back, head, neck and shoulders.
